    Hey everyone -

    I have a form exactly like the first one you see on my home page in the very first section (http://www.b2bleadgenguy.com/). The form is on another landing page, and I don't want to skew the stats there.

    The form lets them request a free report, and I also send some additional content. How would I include a checkbox that says something like "Yes, please subscribe me to your newsletter?"

    Is there a WP plugin or simple code for that?

    Yes, there are definitely easy to use, easy to customize WP plugins for this kind of thing. Here's a link:

    I would use Gravity Forms with the MailChimp Addon for that (assuming you are using MailChimp).

    I am using that feature on my new website 001020.com -> scroll to "Save Money With Your Website" and click get the ebook! Notice the option to sign up to the newsletter.
