My favicon is sometimes missing, but not consistently. I've recently switched to Genesis and the Magazine Pro theme, and the favicon is set in the Customizer.
What I find particularly odd is that when it doesn't show, if I look at the page source, the code is there.

The favicon is displaying fine. You may be running into a caching issue. Also, with WordPress, it's not necessary to use a favicon any longer. The WordPress site identity replaces the favicon and provides you with site tiles for mobile devices.
