eNews Extended: How to Manually Insert Forms Properly?

How can I manually insert those eNews forms down the pages? I managed to extract the code and paste it, but the "E-Mail Address" label is bugged. For example:

If you try to register in the eNews form here, you click on the "E-Mail Address" and the text disappears. But if you try to do the same in the pages where I manually put the code, for example here, the "E-Mail Address" text doesn't disappear.

How can I fix this? Is there anything in the code?
