Do any themes have a sign-up banner that include name and email similar to http://kriscarr.com/?

From my own brief research: you typically use a plugin with the theme of your choice, some examples:
blog subscription (jetpack)
contact form 7
membership

depends on what you are trying to accomplish
For my sites I want to force visitors to sign up once for free admission henceforth
This does not look like wht that site is doing.

I don't know of a particular theme with that exact look but it could be easily achieved with any theme. All Genesis themes use the Genesis Enews Extended widget plugin which can be modified using CSS.
