    Using the Outreach theme, theme settings/Default Layout is set for the first choice, a right sidebar;

    I've used the Default setting as the page template but none of my pages display a right sidebar;

    why isn't a sidebar showing?

    The home page does not use the Primary Sidebar. The internal pages uses the sidebar and it doesn't look like you have anything dragged over into that area.
