    I've been using the Eleven40 child theme for quite awhile now, and I've pretty much used the design that is right out of the box, with the dark thin bar across the top and the site name in that bar on the left. I've done a little tailoring so that I could put a small graphic in the location where the site name shows up by default.

    I was just looking at the showcase here - http://www.studiopress.com/theme/eleven40 - though, and all of those examples look totally different. Is that just a matter of having a skilled designer do extensive customizing? I like the look of some of those.

    Yes, you can get those looks too if you wanted them. You would just reach out to {someone} and tell them what you wanted and they could customize it for you.
