I'm designing a client's site, and they want their email opt-ins to look a certain way that I can't figure out. I need this done within the next couple of days, if not today.
I'm willing to pay for your help (if it's reasonable).
I'm using Minimum Pro Theme and attached are how I need the two opt-ins on the home page to look.

Usually I don't try to answer questions, because I'm not a highly skilled developer, but since you haven't gotten any responses yet, I'll say this:
I discovered a great plugin by the name of "Magic Action Box" the other day.
I haven't used it yet, but I watched a tutorial that showed how to use it (even showed how to use it connected with MailChimp)...a very nice tutorial:
The tutorial shows how to customize the boxes...and it seems that there is a high degree of customization possible.
