The printable brochure image I made custom in photoshop. There is a text on a path tool in PS but I just used a separate layer rotated it and then plapped it on top with a few extra tweaks.
To put it in the header area like that I edited the header code and the ad code if I remember right. I remember I had to tweak the code in the stylesheet a few times to get it all to blend and size just right. Use firebug to check out the code.

I have a support ticket in with studiopress.
With firebug I can see there is an indent and Im wondering if I edit the plugin itseld if that will fix.
