MemberThanks, Brad!
For those who are as much a newbie as me, here's a bit more info. The html can be entered into the "Text To Show After Form:" (and I imagine in the other boxes, as well. The confusing part for me was the word "text" (I didn't realize this could also mean html).
Best,

MemberI found a video that showed me how to use Conditional Tags and add, e.g., is_page(array(39,48,52)) in the Widget Logic box that shows up at the bottom of my sidebar widget info. So that was good!
Unfortunately, for some reason, something keeps going wrong with the background color of the widget (Sidebar); I choose white, see white when I preview, but then the background turns to black after I save (I'm using Design Palette Pro). So this may be a separate problem...

MemberI have the same problem asthibaultsa. I want to delete the side bar widget on my home page, but keep it on the rest of my pages. I installed the Widget Logic plugin as recommended by Susan.
Unfortunately, I can't figure out how to use it!
Is there any more guidance anyone could provide for a newbie?
Thank you!

MemberThank you, Victor. I was missing the part about having to actually create the page before the Static page option appears in the Settings>Reading screen. I was looking for the static option during the Add Page process.
Onward!
Thanks, again,

MemberThank you, braddalton.
Before I saw your response, I adjusted the code I had in the "hidden fields" (basically got rid of some code (don't know how I did it, I just tried to adjust the end </div>) and it seemed to work.
I don't think what I saw at your link talked about how to add a third box (for name).
Any help on this would still be appreciated! (Perhaps I should have a separate post for this remaining query?)
Thank you!
