I have the Yoast SEO plugin installed on my Outreach site and am very happy with it, however the formation of the Home Page still puzzles me, and where you actually set the SEO elements.

This page is the most important page on my site and I can't see where I can configure a description for this page?

The Site title is being pulled from Site Title + Tag Line (Which I would like to be able to do independently

I am pretty sure other themes form the home page in the same manner, so if anyone is running the Yoast Plugin and knows how to set the the Title tag and description I would like to know

I've not used this plugin, but since no-one else has responded to your question yet, I suggest that you head over to Yoast's site, and ask over there:
