    I need to produce an accreditation form on my site asking c30 questions and allowing the reader to reply. Some replies will be tick boxes.

    The material is not for publication after it has been completed.

    Any thoughts for a decent plugin for doing this? I have Premise.

    At present the form is a pdf to be printed out and sent back.

    Well, you could use something like Wufoo.com to create the form. The forms are secure (https) already. The information will be sent to you in email and captured in a downloadable spreadsheet or CSV format. Another alternative would be to use Gravity Forms and build the form that way - but doing it that way would require an SSL certificate because I am sure you would want the respondents responses to be sent securely.
