Ultimate Social Media PLUS plugin is being used for social icons
    >widget in primary sidebar

Displays on home page, but not any posts or pages
    For example: http://www.jimcockrum.com/blog/2015/10/16/the-money-is-in-the-list-right-not-necessarily/

Have tried placing in footer, but doesn't work on posts/pages either.

I can put a new Text widget in the primary sidebar and it works everywhere.

    Any ideas?

I'm seeing the following console warning, which may be why it's not showing:

Given URL is not allowed by the Application configuration: One or more of the given URLs is not allowed by the App's settings. It must match the Website URL or Canvas URL, or the domain must be a subdomain of one of the App's domains.

That is coming from a Facebook JS call, so I'm guessing the app you set up for the plugin is missing a setting somewhere to allow any URL within your domain.
