I think what I am seeking is an INLINE OPT-IN FORM" plug in to attach to a button. Any suggestions?

This is a continuation an earlier posting, I feel I may have asked for the incorrect solution or used the wrong vocabulary:

How do I make a button, when clicked causes a pop-up asking for NAME / EMAIL?
