How do you change "Posted on" with "Last update on"

Hi there,
I have seen this on a blog today but can't remember which one...

In the meta info on top of the post they had:
Last updated on: xx,xx,xx by Author Name

That's awesome because I keep updating my posts but they still display the original date.

Is there any easy way to do that and swap "posted on" with "last updated on"?

Of course I am not just asking to replace the words! But that if for example today I change an image in the post and click "update", the "last updated on" date changes to today.

If next week I change 1 word in the post and click "update" the "last updated on" date changes to next week.
