I am looking for a Theme or Plugin that will enable me to capture some enquiries coming to a magician booking website.

I want the user to hit a very simple page with no menus and just the option to enter some very basic information.

Landing Page - Question 1: Where in the UK are you looking to hire a magician?

This takes you to next page / screen - Is this for a wedding / party / etc

Next Screen - What is your budget,

What is the date of your event.

This then says - we are sending your enquiry to all the magicians on our books that match your enquiry and you should receive responses from them in the next few days.

Does anyone have any ideas or tips on how I could achieve this?

Just needs to be clean and simple

    Thanks
    Roger

There are Multi-Page Forms in Gravity Forms which is a paid plugin.
There is a non WP solution in Type Form that may be able to be used in an i Frame.
