Enews Extended widget not showing form

I'm having a problem with the latest Magazine Pro theme and the enews extended plugin. I'm sure I have everything set up correctly. The problem is that the widget does not display the form. I've installed this widget on other genesis themes a few times with no problems. I've followed the installation guide to a "T" but I still have the same problem

I've tried deactivating and reactivating the plugin and theme also. Has anyone run into this problem before?

I feel your pain Jlhill, Does anyone have a solution for this? There is obviously a bug.

I see the plugin is working, just that you need to fill out the widget box empty box's.

I am using the same theme btw.
