Has anyone created an online assessment like this? http://www.outofservice.com/bigfive/ I'm using parallax theme.

You could do that with Gravity forms or Google Docs.

Thanks Brad! Have you used Gravity forms?

I talked with Gravity forms and they said they could do it. However, it would require some php programming for calculations and to provide answers at end.
