    On my executive themed site, I want to create a contact form for some sidebars (using the simple sidebars plugin) but i want it to look just like the newsletter widget but have a few fields like name, email, phone and it send an email. I tried messing around with Contact Forms 7 and trying to duplicate the styling but it didnt look right.

    Any suggestions?

    Jetpack includes a nice contact form.

    If you don't want to install the plugin, download it and drag the contact form module from within Jetpack and install the contact form on its own.

    Otherwise Gravity forms is the best form plugin.

    You'll also need to add support for shortcodes in your child themes functions.php file:

    add_filter('widget_text', 'do_shortcode');
    add_filter('widget_title', 'do_shortcode');
